Collaborative Documentation is an evidence informed clinical practice in
which the clinician documents all aspects of the clinical record with the consumer present. The
consumer is privy to all documentation content and is afforded the opportunity to have their
perspective documented as well. Collaborative Documentation has multiple demonstrated benefits
